    Effect of HBXIP on biological function and PI3K/Akt signaling pathway of adenoid cystic carcinoma cell line ACC-M

    To study the effect of hepatitis B virus X protein binding protein (HBXIP) on proliferation, migration and invasion of adenoid cystic carcinoma cell line ACC-M, and the possible mechanism of PI3K/Akt signaling pathway. METHODS: HBXIP plasmid was transfected into ACC-M. The cells were divided into experimental group (transfected with plasmid pEGFP-N1-HBXIP) control group (non-transfected group) and blank control group (vector group, pEGFP-N1). RT-PCR was used to detect the expression HBXIP in ACC-M; MTT assay, transwell chamber experiments and scratches over the proliferation of HBXIP were utilized individually to evaluate the influence of HBXIP on ACC-M expression, migration and invasion; Western blotting was used to detect the protein expression of Akt, p-Akt, PI3K, p-PI3K and S100A4 after overexpression of HBXIP. Statistical analysis was performed using SPSS 18.0 software package. RESULTS: MTT results showed that the number of surviving cells of experimental group was significantly higher than the control group (P<0.05); Scratch test results showed that the cell mobility of the experimental group was significantly higher than the control group (P<0.01); Transwell chamber experiments showed that the number of cell invasion of the experimental group was significantly higher than the control group (P<0.01); Western blotting results showed that compared with the control group, the expression of p-Akt, p-PI3K and S100A4 in the experimental group with overexpressed HBXIP was relatively increased. CONCLUSIONS: Overexpression of HBXIP gene promotes ACC-M proliferation, invasion and migration. Further, ACC-M proliferation, invasion and migration may be promoted by increased Akt, PI3K phosphorylation and S100A4 protein expression.

    Early orthodontic intervention remains a controversial notion in current dental care regime. Whilst early orthodontic treatment for children is the province for orthodontic specialists, a growing number of general practitioners, who may not possess sufficient specialty knowledge, are also involved, raising the concerns about the propriety and quality of their treatment modalities. However, orofacial development of children and adolescents is in a very complicated environment. Comprehensive theoretical knowledge and a great wealth of practical experience are required to ensure the final treatment effect. The possible complications should be kept under control and fully informed to patients and their parents. In order to unify and standardize early orthodontic treatment protocol and help to promote healthy and orderly development of early orthodontic treatment, this consensus summarized the practical experience of orthodontic experts from many famous colleges and affiliated hospitals for clinical reference.

    Research progress and current situation of oral surgery robots based on published papers and patents analysis

    PURPOSE: To analyze the research progress and technological development of oral surgery robots, reveal the future development trend of oral surgery robots and put forward policy suggestions. METHODS: Focusing on the topic of oral surgery robots, literature review and patent data retrieval methods were used to analyze data from 2011 to 2023 from the perspectives of quantity, source, and technical field. RESULTS: The number of academic papers and patent applications in the field of oral surgery robots had explosive growth since 2015. The literature mainly focused on topics such as oral implantation and digitalization, with Chinese scholars publishing the most papers. The patents focused on tools such as models and instruments dedicated to oral surgery, as well as technical fields such as implant visual navigation and positioning systems. CONCLUSIONS: Oral surgery robots should be developed towards miniaturization and specialization in the future, and domestic breakthroughs in core technologies such as special instruments need to be accelerated.

    PURPOSE: To explore the expression of hub genes, pathways and inhibitory immune checkpoints of CD8+ T cells in metastatic lymph nodes of head and neck squamous cell carcinoma(HNSCC) in C3H/He mice. METHODS: A popliteal lymph node metastasis model of immunocompetent C3H/He mice was constructed with SCC-7 cell line of HNSCC, and the lymph node metastasis status was determined by immunofluorescence. CD8+ T cells in normal and metastatic lymph nodes were sorted by flow cytometry, and transcriptome sequencing analysis was performed to screen out differentially expressed genes. GO functional enrichment analysis and KEGG pathway enrichment analysis were performed sequentially. Flow cytometry and multiplex immunohistochemical were used to detect the expression of four immune checkpoints of CD8+ T cells in metastatic lymph nodes of tumor-bearing mice. SPSS 26.0 software package was used for statistical analysis. RESULTS: After 4 weeks of foot pad injection of SCC-7 cells, C3H/He mice displayed obvious metastasis in popliteal drainage lymph node. A total of 912 differentially expressed genes were screened out by transcriptome sequencing analysis of CD8+ T cells in normal and metastatic lymph nodes, including three inhibitory immune checkpoint-related genes which were upregulated(Pdcd1, Lag3 and Tigit). Eight tumor-related signaling pathways were screened out by KEGG network analysis, including Toll-like receptor signaling pathway, NF Kappa-B signaling pathway, TNF signaling pathway, MAPK signaling pathway, IL-17 signaling pathway, NOD-like receptor signaling pathway, PD-L1 and PD-1 immune checkpoint pathway in tumors and p53 signaling pathway. Flow cytometry showed high expression level of PD-1 and TIM-3 in CD8+ T cells in mouse metastatic lymph nodes. It was further confirmed by multiplex immunohistochemical that PD-1 was highly expressed in CD8+ T cells in metastatic lymph nodes of patients with HNSCC. CONCLUSIONS: Tumor-related signaling pathway of CD8+ T cells in mouse metastatic lymph nodes is significantly activated. The PD-1 expression level of CD8+ T in metastatic lymph nodes of mice and patients with HNSCC is markedly increased. Immunotherapy targeting CD8+ T cells may become a new strategy for the prevention and cure for lymph node metastasis of HNSCC.

    The orthodontic therapeutic principles for malocclusions concurring with temporomandibular joint disorders

    The biological nature of temporomandibular joint (TMJ) featuring adaptive remodeling allows for TMJ structural changes in response to external stimuli, including alterations in occlusion and in mandibular posture. Correlative susceptibility exists between malocclusions and TMJ disorders (TMD), evidenced by high incidence of TMD coinciding with skeletal originated malocclusion with facial prognathsim (Angle Class I skeletal II high angle). Clinical manifestations include markedly retruded mandible together with increased overjet, or mandibular transverse deviation, due to condylar pathological resorption. It is critically imperative to execute differential diagnosis between the routine malocclusions and those infringed with TMD. The prime stipulations governing the orthodontic procedures include the patient's notification of the existential TMD conditions prior to any orthodontic interventions, close inspections of TMD developments during the treatment, and a specially designed retention protocol aftermath. For the cases where severe overjet and mandibular displacement occur as a result of TMD, some specific orthopedic modalities can be implemented to improve the occlusion by restoring the shifted mandible. TMD conditions are also expected to be relieved with mandibular repositioning, which encourages new bone formation in the degraded condyles. The newly developed clear version of orthopedic appliances, namely, S8-SGTB, S8-SGHB, S9 and S10 are of promising efficacy in treating this category of patients.

    PURPOSE: To analyze the difference of metabolites of tongue coating between patients with intra-oral halitosis and healthy individuals by untargeted metabolomics, and to explore significant differences in metabolites of intra-oral halitosis as biomarkers. METHODS: The untargeted metabolomics of tongue coating samples from 12 patients with intra-oral halitosis and 12 healthy individuals were studied by liquid chromatography and mass spectrometry combined with principal component analysis and orthogonal partial least squares discriminant analysis. The value of variable importance in projection >1 and P<0.05 of Student's t test in the orthogonal partial least squares-discriminant analysis model were used as the criteria to screen and determine the differential metabolites. RESULTS: There were differences in the metabolites of tongue coating between patients with intra-oral halitosis and healthy individuals, and 11 different metabolites were identified. They were valyl-arginine, glycine-phenylalanine, tryptophyl-proline, deoxyadenosine, 4,5-dihydroniveusin A, N-acetyl-DL-tryptophan, paramethasone acetate, cyclopentanol, [(2-hexylcyclopentylidene) amino]thiourea, L-pipecolic acid and taurine. In the intra-oral halitosis group, the expressions of Glycine-phenylalanine and N-acetyl-DL-tryptophan were significantly up-regulated, while the expressions of taurine were significantly down-regulated. CONCLUSIONS: There are differences in the metabolites of tongue coating between patients with intra-oral halitosis and healthy individuals. The differential metabolites with diagnostic value may be used as diagnostic markers of intra-oral halitosis.

    Clinical efficacy of the combination of miniscrew with clear aligner in controlling the roller coaster effect

    PURPOSE: To investigate the clinical efficacy of miniscrew in controlling the roller coaster effect in long-distance space closure with clear aligner. METHODS: Sixteen adult patients with bimaxillary protrusion were selected. All cases were treated with clear aligner and their four first premolars were extracted. G6 was designed in all cases. In the experimental group, four miniscrews were inserted at the beginning of treatment to control the teeth in three dimensions; while in the control group, the miniscrews were not applied until obvious roller coaster effect appeared. Paired t test was used to analyze the data with Graphpad Prism 6.0 software package. RESULTS: All the extraction space was closed successfully and the lateral profile was improved significantly. The torque of the upper incisor was under well control and no roller coaster effect happened in the experimental group; while in the control group, the lingual inclination of the upper incisors and open bite of the posterior teeth developed(P<0.05). After the miniscrews intrusion of the upper anterior teeth, the occlusion relationship improved significantly. CONCLUSIONS: Miniscrews effectively prevents and eliminates the roller coaster effect in the process of long-distance space closure with clear aligner.

    Analysis of the possible causes of endodontic treatment failure by inspection during apical microsurgery treatment

    PURPOSE: To analyze the possible causes of previous endodontic treatment failure by microscopic inspection during apical microsurgery. METHODS: Two hundred and eighty-nine teeth of previous endodontic treatment failure were collected from patients in Shanghai Xuhui District Dental Center, between January 2006 and January 2014. All surgical procedures were performed by using an operating microscope, and 238 roots were included in the study. The surface of the apical root to be resected or the resected root surface after methylene blue staining was examined during the surgical procedure and inspected with 26 magnification to determine the state of the previous endodontic treatment by using an operating microscope. Fisher's exact test was used to analyze the data with SPSS 19.0 software package. RESULTS: Among the 238 roots with periapical surgery, analysis of the reasons for previous endodontic treatment failure included leaky canal (29.41%), missing canal (15.55%), underfilling (15.55%), anatomical complexity (7.98%), overfilling (4.20%), apical fenestration (4.20%), iatrogenic problem (3.36%), apical calculus (2.52%), apical cracks (1.68%) and unknown reasons (15.55%). The frequency of possible failure causes and tooth position were closely correlated (P<0.001). CONCLUSIONS: Apical microsurgery can better inspect possible causes of previous endodontic treatment failure, in order to improve the success rate of endodontic treatment.

    Gene expression profiling analysis of stress-sensitive genes and their potential functions in myoblasts

    PURPOSE: To screen the stress-sensitive genes in myoblasts and reveal the potential target genes and their regulatory mechanisms of facial muscle remodeling induced by functional orthopaedic force. METHODS: The procedure involved the use of gene microarray technology to identify the differentially expressed genes(DEGs) in myoblasts. DEGs were then categorized by Gene Ontology(GO) and Kyoto Encyclopedia of Genes and Genomes(KEGG) enrichment analyses. Furthermore, real-time quantitative PCR(qRT-PCR) was used to verify the DEGs. Western blot, transmission electron microscopy(TEM), and confocal laser scanning microscope(CLSM) were employed to detect the effect of stress on autophagy in myoblasts. The data were analyzed by SPSS 17.0 software package. RESULTS: A total of 1 410 DEGs were identified in stretched myoblasts, with 788 up-regulated and 622 down-regulated genes. GO enrichment analysis indicated that DEGs were primarily involved in signal transduction, biopolymer metabolic process, and protein metabolic process. KEGG analysis revealed that DEGs were primarily associated with ECM-receptor interaction, pathway in cancer, MAPK signaling pathway, focal adhesion and lysosome. Both TEM and CLSM showed that stress could promote the formation of autophagosomes, and Western blot demonstrated that stress could promote the expression of autophagy-related molecules Beclin-1 and LC3-II. Rapamycin could enhance all the above processes, while 3-MA could inhibit them. CONCLUSIONS: Autophagy may play an important role in the regulation of myoblast fate induced by cyclic tensile stress.

    Comparison of aesthetic effect between immediate and delayed implant restoration of single maxillary anterior tooth

    PURPOSE: To compare the aesthetic effects of immediate implant restoration and delayed implant restoration of single maxillary anterior tooth. METHODS: A total of 163 patients who underwent single anterior tooth implantation in the maxilla at Taikang Xianlin Drum Tower Hospital from January 2020 to May 2023 were selected. They were divided into immediate implant restoration group(n=75) and delayed implant restoration group(n=88). In the immediate implant restoration group, a temporary implant was installed and implanted immediately after extraction, while in the delayed implant restoration group, the implant was implanted 3-6 months after extraction and repaired after the implant was stabilized. The outcomes, including implant success rate, soft tissue aesthetics, implant stability, aesthetic outcomes, periodontal health, pink esthetic score (PES), white esthetic score (WES), bone resorption after 12 months and subjective satisfaction were compared between the two groups. RESULTS: After exclusion, totally 62 patients in the immediate implant restoration group and 64 patients in the delayed implant restoration group were included in the analysis. Both groups had a 100% implant success rate with no significant difference between the two groups(P>0.05). The immediate implant restoration group was significantly better in terms of soft tissue aesthetics and implant stability compared to the delayed implant restoration group(P<0.05). Aesthetic effect scores for the immediate implant restoration group were significantly higher than those of the delayed group at 1, 3, 6 and 12 months postoperatively(P<0.05). The periodontal tissue health of the immediate implant restoration group was significantly better than that of the delayed group at various time points(P<0.05). PES and WES scores of immediate implant restoration group were significantly better than those of the delayed group(P<0.05). At 12 months after surgery, bone resorption in the immediate implant restoration group was significantly lower than that in the delayed group, subjective satisfaction score was significantly higher than that in the delayed group(P<0.05), and there was no significant difference in color satisfaction between the two groups(P>0.05). CONCLUSIONS: Immediate implant restoration of maxillary single anterior teeth is superior to delayed implant restoration in soft tissue aesthetic effect, implant stability and patient satisfaction, which is suitable for patients with high aesthetic requirements.

    Segmentation and validation of mandibular canal and its bifurcation on cone beam CT based on deep learning

    PURPOSE: To train the U-net of convolutional neural network to establish a method for detecting and segmenting the mandibular canal and its bifurcation, and validate its accuracy based on the ground truth labeled by experts. METHODS: A total of 290 CBCT scans were collected from Shanghai Stomatological Hospital from Jan. 2022 to Dec. 2022, which were divided into training set of 200 scans and test set of 90 scans. Model training included two steps. In the first step, bilateral mandibular canals and its bifurcation of 50 CBCT scans were labeled in 3D Slicer image computing platform by investigators. Three dimensional U-net segmentation model were trained initially with data enhancement. A morphological post-processing method was applied to the predicted results. In the second step, pseudo label method was employed to help annotating the mandibular canal and corresponding bifurcations on remaining 150 CBCTs, which would be included in training set after revision. Three dimensional U-net model was trained based on these 200 data. During test phase, totally 90 scans were labeled by two doctors and U-net model respectively. Consistency check was conducted to evaluate the labels between two doctors. Dice similarity coefficient and Hausdorff distance were calculated to evaluate the labels between doctors and the model. The detection rate of bifurcation was calculated. SPSS 20.0 software package was used for data analysis. RESULTS: In 90 CBCT test set, the Kappa value between two dentists' annotations was 0.667. The average Dice and Hausdorff distance between predictions and labels of doctors were (0.739±0.068) and (0.988±1.14) mm. In bifurcation detection, the detection rate was 91.30% on scans with clear bifurcations. CONCLSIONS: The dentification and segmentation U-net model of mandibular canal on dental CBCT can be reliable and practical for its high segmentation precision and predicting speed.

    The impact of oral occlusal exercises on occlusal performance of oral cancer patients following segmental mandibulectomy without simultaneous reconstruction

    PURPOSE: To investigate the effects of oral occlusion exercises on occlusal performance of oral cancer patients who underwent segmental mandibular resection without simultaneous reconstruction. METHODS: From October 2022 to May 2023, a total of 63 oral cancer patients who underwent segmental mandibular resection without reconstruction at the Department of Oromaxillofacial Head and Neck Oncology, Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine were selected. They were randomly divided into control group (30 patients) and experimental group (33 patients). The control group received conventional care, while the experimental group underwent oral occlusion exercises except conventional care. After three months, the maximum total occlusal force(TOF), asymmetry index of occlusal force(AOF), center of occlusal force(COF), and mastication efficiency of both groups were compared. Statistical analysis was conducted using SPSS 26.0 software package. RESULTS: Compared with the control group, TOF in the experimental group was increased, AOF and COF were decreased, and the differences were statistically significant(P<0.01). The masticatory efficiency of the experimental group was significantly better than that of the control group(P<0.01). CONCLUSIONS: Oral occlusal exercises can improve occlusal performance in patients with mandibular resection without reconstruction, but larger sample size and rigorous design are needed to further verify its clinical effect and mechanism.

    Clinical application of individualized three-dimensional printing implant template in multi-tooth dental implantation

    To study the value and satisfaction of three-dimensional printing implant template and conventional implant template in multi-tooth dental implantation. METHODS: Thirty cases (83 teeth) with missing teeth needing to be implanted were randomly divided into conventional implant template group (CIT group, 15 cases, 42 teeth) and 3D printing implant template group (TDPIT group, 15 cases, 41 teeth). Patients in CIT group were operated by using conventional implant template, while patients in TDPIT group were operated by using three-dimensional printing implant template. The differences of implant neck and tip deviation, implant angle deviation and angle satisfaction between the two groups were compared. The difference of probing depth and bone resorption of implant were compared 1 year after operation between the two groups. The difference of success rate and satisfaction of dental implantation were compared 1 year after operation between the two groups. SPSS19.0 software package was used for statistical analysis. RESULTS: The deviation direction of the neck and the tip in disto-mesial, bucco-palatal, vertical direction and angle of implants in disto-mesial and bucco-palatal direction in TDPIT group were significantly lower than in CIT group (P<0.05), and angle satisfaction of implants in disto-mesial and bucco-palatal direction in TDPIT group were significantly higher than in CIT group (P<0.05). There was no significant difference between the two groups in the probing depth and bone resorption of implants 1 year after operation (P>0.05). The difference of the cumulative success rate in dental implantation at 3 months and 6 months between the two groups were not significant (P>0.05), but the cumulative success rate of TDPIT group was significantly higher than CIT group at 9 months and 1 year (90.48% vs 100%, χ2=4.102, P=0.043). The patients' satisfaction rate of dental implantation in TDPIT group was significantly higher than in CIT group (86.67% vs 53.33%, χ2=3.968, P=0.046). CONCLUSIONS: Using three-dimensional printing implant template can obtain better accuracy of implant, higher implant success rate and better patients' satisfaction than using conventional implant template. It is suitable for clinical application.
