    Relationship between upper airway and cranial-facial growth patterns in adolescents with normal occlusion

    PURPOSE: To investigate the relationship between upper airway and cranial-facial growth pattern among teenagers with normal occlusion. METHODS: The study subjects consisted of 52 boys and 64 girls with normal occlusion. Their cephalometric radiographs were obtained. The sagittal diameter of upper airway was analyzed according to gender and different growth pattern. The relationship of upper airway diameter and cranial-facial formation was evaluated by stepwise regression analysis using SPSS 21.0 software package. RESULTS: There was no significant difference between male and female about upper airway diameter in teenagers, also no difference was found among different growth patterns. Regression analysis showed that upper airway was associated with some cranial-facial formation parameters. CONCLUSIONS: Although there is no significant difference in the upper airway diameters among different growth patterns, there is still certain association between cranial-facial formation and upper airway in adolescents.

    Orofacial myofunctional therapy improves facial morphology of children with obstructive sleep apnea after adenotonsillectomy

    PURPOSE: This study investigated the effectiveness of orofacial myofunctional therapy(OMT) in improving facial morphology of children with obstructive sleep apnea (OSA) after adenotonsillectomy (AT). METHODS: Ten children aged from 4-7 years with persistent oral breathing for more than 1 month after adenotonsillectomy were chosen to receive orofacial myofunctional therapy. The patients were required to take photos before and after orofacial myofunctional therapy. In order to compare the soft changes before and after OMT treatment, twelve representative mark points were selected and used for proportion and angle measurements. Graphpad Prism 8 statistical software was used for statistical analysis, to compare the differences in facial morphology of patients before and after treatment. RESULTS: Compared with before OMT, a significant difference was found in the proportion of Sn-Ls/Sn-Stms(P=0.0002), Sn-Stms/Sn-Me'(P<0.05), as well as in the angle of Gs-Sn-Pos (P<0.05), nasolabial angle(P=0.0005), mentolabial angle (P=0.0026) after OMT treatment. CONCLUSIONS: Orofacial myofunctional therapy can be considered as an effective complementary treatment for OSA patients with oral breathing after adenotonsillectomy.

    Clinical study of the effect of fixed Twin-block on adolescent skeletal ClassⅡmalocclusion

    PURPOSE: To evaluate the retention ability of fixed Twin-block appliance and its clinical effect on adolescent skeletal ClassⅡmalocclusion. METHODS: Twenty-six skeletal ClassⅡdivision 1 adolescents (M:12, F: 14; age:11~13 years, average: 11.8 years) were chosen and fixed Twin-block appliance was used to guide the mandibular protrusion for one year. Before and after treatment, cephalometric films were taken to observe the skeletal, dental and soft tissue changes. Graphpad Prism 6.0 software was used for Student's t test. RESULTS: There was no loosening or destruction of the appliance during the treatment and the profile of all cases was improved significantly. The indexes that showed significant difference(P<0.05) included the mandibular length and position (Co-Gn, SNB, ANB, Pog-VL, Pos-VL), the inclination and position of the maxillary incisors(U1-VL, U1-SN), the sagittal position of the mandibular dentition(L1-VL, L6-VL). The indexes that showed insignificant difference(P>0.05) included the mandibular plane(MP-SN), the length and position of the maxilla (SNA, A-VL), the vertical position of the maxillary incisor(U1-HL), the position of the maxillary posterior teeth(U6-VL, U6-HL), the mandibular incisor inclination(IMPA) and the vertical position of the mandibular dentition(L6-MP,L1-MP). CONCLUSIONS: Fixed Twin-block appliance can enhance the mandibular anchorage, effectively promote the mandibular growth and improve the facial profile.

    Early orthodontic intervention remains a controversial notion in current dental care regime. Whilst early orthodontic treatment for children is the province for orthodontic specialists, a growing number of general practitioners, who may not possess sufficient specialty knowledge, are also involved, raising the concerns about the propriety and quality of their treatment modalities. However, orofacial development of children and adolescents is in a very complicated environment. Comprehensive theoretical knowledge and a great wealth of practical experience are required to ensure the final treatment effect. The possible complications should be kept under control and fully informed to patients and their parents. In order to unify and standardize early orthodontic treatment protocol and help to promote healthy and orderly development of early orthodontic treatment, this consensus summarized the practical experience of orthodontic experts from many famous colleges and affiliated hospitals for clinical reference.

    Application of one-piece implant-supported detachable telescope retained fixed bridge in edentulous cases

    PURPOSE To evaluate the clinical effectiveness of one-piece implant-supported detachable telescopic fixed bridge in edentulous patients. METHODS Seventeen patients were treated with one-piece implant-supported detachable telescopic fixed bridge. A total of 18 prostheses were fabricated with 8 in the upper jaws and 10 in the lower jaws.Fixed bridges retained by telescopic crowns were used as final prostheses, with milling titanium or all-ceramic abutments as primary crowns, gold-electroforming crowns as secondary crowns. Surveys about clinical and radiographic examination, satisfaction and prosthetic complications were conducted after 3 months,1 year, 2 years, 3 years after final rehabilitation. Data analysis was performed using SPSS 17.0 software package.RESULTS Radiography showed stable bone levels for all implants except 2 implants, which were observed slight marginal bone resorption. The results of one-way ANOVA showed that no significant difference in modified plaque index or modified sulcus blooding index was found during the follow-up period (P>0.05). The probing attachment level deteriorated by 1.5 mm during the first 3 years (P<0.05). Eighteen restoration provided sufficient fixation and stability. Two porcelain fractures occured but had no influence on restoration. The patients were highly satisfied with the outcomes. The frequency of prosthetic maintenance per patient per year was 0.11.CONCLUSIONS One-piece implant-supported detachable telescopic fixed bridge is an effective method with satisfactory long-term aesthetic and stable outcomes in edentulous patients.

    The effect of human PBMC on the growth of oral squamous cell carcinoma cell

    PURPOSE: To explore the effect and mechanism of activated peripheral blood mononuclear cell (PBM) on the growth and apoptosis of oral squamous carcinoma cells in vitro. METHODS: ELISA, RT-PCR, MTT and flow cytometry were used to detect cytokines secretion and oral squamous carcinoma cell growth and apoptosis affected by activated PBMCs with PHA. The data was analyzed using SPSS17.0 software pakage. RESULTS: The cytokines secretion, mRNA expression of granzyme B and perforin, inhibition of growth and induction of apoptosis of oral squamous carcinoma cells treated by the activated PBMCs with PHA were significantly higher than the control groups (P<0.05). CONCLUSIONS: The human PBMC can be activated effectively with the stimulations of PHA, which can significantly inhibit growth and induce apoptosis of oral squamous carcinoma cells in vitro.

    Assessment of pulpotomy in cariously exposed permanent teeth

    To observe the clinical effect of pulpotomy for cariously exposed permanent teeth with closed apices and provide guidance for clinical application. METHODS: Sixty-two cariously exposed permanent teeth with closed apices and vital pulps were included. Preoperative pulpal and periapical diagnosis was established. The teeth were divided into two groups, the experimental group was treated with pulpotomy(32 cases), and the control group was treated with one-visit root canal therapy (one-visit RCT) (30 cases). Clinical and radiographic evaluation was completed at 1 month, 3 months, 6 months, 12 months postoperatively. Statistical analysis was performed using SPSS 18.0 software package. RESULTS: After 1 year, the success rate of pulpotomy was 93.75%, and the success rate of root canal therapy was 93.10%. There was no significant difference between pulpotomy and root canal therapy( χ2=0.010,P=0.665). CONCLUSIONS: The advantages of pulpotomy are less invasive and less tooth destruction. It is a successful treatment option for cariously exposed pulps in mature permanent teeth.

    Clinical effect of digital impression combined with all-ceramic denture on restoration of 60 patients with dental defects

    PURPOSE: To analyze the clinical effect of digital impression combined with all-ceramic denture restoration on patients with dental defects. METHODS: A study was conducted on 120 patients with dental defects admitted to Dongfeng Stomatological Hospital from June 2018 to June 2020. The two groups of patients were randomly divided into digital imprinting modules and silicone rubber imprinting modules using a random number table method. There were 60 cases in each group. The silicone rubber imprint module used traditional silicone impression combined with all-ceramic denture restoration, while the digital imprint module used digital impression combined with all-ceramic denture restoration.The clinical efficacy of the two groups was observed. Gingival index (GI), periodontal index (PI) before dental restoration, during dental restoration and 6 monthes after dental restoration were compared. The adjacent surface contact conditions, occlusion and adverse reactions of the two groups were compared. SPSS 22.0 software package was used for statistical analysis. RESULTS: The two groups of patients selected grade A and grade B for the evaluation of the restoration when the restoration was completed. The number of patients who chose grade A for the digital imprint module was more than that of the silicone rubber imprint module,but there was no significant difference(P>0.05). Six months after the tooth was worn, GI and PI indexes of the two groups of patients increased. GI and PI indexes of the silicone rubber stamping module were significantly higher than those of the digital stamping module(P<0.05). When dental restoration was completed and 6 months after the tooth was replaced, the pass rate of contact between the adjacent surfaces of the imprinting module patients was significantly higher than that of the silicone rubber imprinting module(P<0.05). When dental restoration was completed, the occlusion of the digital imprinting module patients was significantly better than that of the silicone rubber imprinting module(P<0.05). Six months after wearing the denture, there was no significant difference in occlusion between the two groups of patients(P>0.05). When dental models of the two groups of patients were taken, the incidence of adverse reactions in patients with digital imprints was significantly lower than that of silicone rubber imprints(P<0.05). CONCLUSIONS: The use of digital impressions combined with all-ceramic restorations to repair patients with dental defects can effectively improve the treatment effect, improve prognostic GI and PI indexes of the patients, increase the pass rate of the adjacent surface contact and occlusion of the tooth, and reduce the process of dental restoration. The incidence of adverse reactions are minimal, with good prognostic effects. It is worthy of clinical application.

    Effectiveness of 3 different methods in prevention of dental caries in permanent teeth among children

    PURPOSE: To compare the effectiveness of fluoride vanish, fluoride foam, pit and fissure sealant with fluoride in prevention of dental caries, and investigate appropriate approach to prevent dental caries at schools. METHODS: A randomized controlled clinical trial with 4 parallel groups was conducted. Totally 1016 children from 4 schools and 33 classes with a mean age of 7 to 8 years were included. They were randomly allocated into 4 groups: ① fluoride vanish, semi-annual application; ② fluoride foam, semi-annual application; ③ resin sealant, single placement; and ④ placebo control with oral hygiene instruction. Follow-up examinations were conducted after 2 years to monitor dental caries increment of first molars among these children. Data analysis was performed using SPSS 18.0 software package. RESULTS: Two years later, 977 children (96.2%) were examined. The results of multiple factor ANCOVA showed that fluoride vanish and resin sealant had significantly lower mean dental caries increment on tooth surface compared with control group (P<0.05). No significant difference of mean dental caries increment on tooth surface was found between fluoride foam and control group (P>0.05). CONCLUSIONS: The placement of resin sealant with fluoride and annual application of fluoride protector are effective in prevention of dental caries in permanent teeth of children.

    PURPOSE: To investigate the clinical efficacy of miniscrew in controlling the roller coaster effect in long-distance space closure with clear aligner. METHODS: Sixteen adult patients with bimaxillary protrusion were selected. All cases were treated with clear aligner and their four first premolars were extracted. G6 was designed in all cases. In the experimental group, four miniscrews were inserted at the beginning of treatment to control the teeth in three dimensions; while in the control group, the miniscrews were not applied until obvious roller coaster effect appeared. Paired t test was used to analyze the data with Graphpad Prism 6.0 software package. RESULTS: All the extraction space was closed successfully and the lateral profile was improved significantly. The torque of the upper incisor was under well control and no roller coaster effect happened in the experimental group; while in the control group, the lingual inclination of the upper incisors and open bite of the posterior teeth developed(P<0.05). After the miniscrews intrusion of the upper anterior teeth, the occlusion relationship improved significantly. CONCLUSIONS: Miniscrews effectively prevents and eliminates the roller coaster effect in the process of long-distance space closure with clear aligner.

    Effect of different surface treatments on the bond strength of fiber posts to resin cement

    PURPOSE: To evaluate the effects of five different surface treatments on the bond strength between fiber posts and resin cement. METHODS: Fifty fiber posts were randomly divided into 5 groups for different surface treatments. Group A was treated with silane coupling agent (Clearfil Porcelain Bond Activator,Kuraray); Group B was treated with silane coupling agent and then coated with dentin bonding agent (Clearfil SE Bond,Kuraray); Group C was immersed in 30%H2O2; Group D was immersed in 30%H2O2 and then treated with silane coupling agent; Group E received no surface treatment (control group). After bonding to resin cement, each group was then divided into 2 subgroups equally,while one group was stored in sodium chloride for 24 h at 37℃, and the other group was stored in sodium chloride for 24 h at 37℃ and then subjected to thermal cycling for 10000 times. Microtensile bond strengths were tested and the data was statistically analyzed using SPSS17.0 software package. RESULTS: The microtensile bond strength before thermal cycling was(6.7±0.7) MPa for group A,(14.4±1.1) MPa for group B,(10.7±0.9) MPa for group C,(16.0±1.0) MPa for group D and (6.7±1.0) MPa for group E. After thermal cycling, the microtensile bond strength was (6.7±0.7) MPa for group A, (13.1±0.7) MPa for group B, (9.0±0.7) MPa for group C, (15.0±0.9) MPa for group D and (5.6±0.7) MPa for group E. The results showed that surface treatments had significant impact on the bond strength (P<0.05). There was no significant difference between group A and group E. Group D had the highest bond strength compared with the other groups. Microtensile bond strengths were significantly different before and after thermalcycling treatment in each group (P<0.05). CONCLUSIONS: Thermocycling treatment decreases the bond strength of fiber posts to resin cement with these 5 surface treatments. Coupled with 30%H2O2 solution and silanization, the bonding strength of fiber posts to resin cement can increase significantly.
