颞下颌关节紊乱咀嚼肌疼痛患者感觉功能定量测量分析

Quantitative measurement and analysis of sensory function in patients with temporomandibular disorder

摘要: 目的: 测量分析颞下颌关节紊乱(temporomandibular disorder,TMD)咀嚼肌疼痛患者在定量感觉功能方面的变化,分析其与患者疼痛感知和功能障碍的关系,为咀嚼肌疼痛患者的个性化治疗提供依据。方法: 选择50例确诊为TMD咀嚼肌疼痛的患者作为试验组,同期招募50例性别、年龄匹配的无肌肉疼痛患者作为对照组。对所有受试者进行全面的定量感觉测试,包括冷感觉阈值(cold detection thresholds,CDT)、热感觉阈值(warmdetection thresholds,WDT)、冷痛觉阈值(cold pain thresholds,CPT)、热痛觉阈值(hot pain thresholds,HPT)和压力疼痛阈值(pressure pain threshold,PPT),并进行疼痛问卷调查和下颌功能障碍指数(mandibular function impairment questionnaire,MFIQ)评估。结果: 试验组患者的CDT、WDT、CPT、HPT和PPT均显著低于对照组(P<0.05),且疼痛问卷评分和MFIQ评分显著高于对照组(P<0.05)。定量感觉测试结果与疼痛问卷评分和MFIQ评分呈负相关。结论: 咀嚼肌疼痛患者在多个感觉维度上表现出阈值降低,表明感觉疼痛现象普遍存在。这些感觉功能异常可能与患者疼痛感知和功能障碍有关。定量感觉测试能为了解咀嚼肌疼痛的病理生理机制提供更多信息,并为个性化治疗策略的开发提供依据。

Abstract: PURPOSE: To measure and analyze the changes in quantitative sensory function in patients with temporomandibular disorder (TMD) and masticatory muscle pain, and to analyze its relationship with pain perception and dysfunction, so as to provide a basis for personalized treatment of patients with masticatory muscle pain. METHODS: Fifty TMD patients with masticatory muscle pain were selected as the experimental group, and 50 gender and age matched TMD patients without masticatory muscle pain were selected as the control group. All participants underwent comprehensive quantitative sensory test, including cold detection thresholds(CDT), warm sensation thresholds(WDT), cold pain thresholds (CPT), hot pain thresholds(HPT), and pressure pain thresholds (PPT). In addition, pain questionnaire surveys and mandibular function impairment questionnaire(MFIQ) assessments were conducted. RESULTS: The CDT, WDT, CPT, HPT, and PPT of TMD patients were significantly lower than those of the control group(P<0.05). The pain questionnaire scores and MFIQ scores of myalgia patients were significantly higher than those of the control group (P<0.05). The results of quantitative sensory testing were negatively correlated with pain questionnaire scores and MFIQ scores. CONCLUSIONS: Patients with masticatory muscle pain show decreased thresholds in multiple sensory dimensions, indicating that sensory pain phenomena are widespread. These sensory function abnormalities may be related to the patients' pain perception and functional disorders. Quantitative sensory test can provide in-depth insights into the pathophysiological mechanisms of masticatory muscle pain and offer a basis for the development of personalized treatment strategies.
