牙列缺损患者血清ICAM-1、MMP-8水平对口腔种植修复预后的影响

Effect of serum ICAM-1 and MMP-8 levels in patients with dentition loss on prognosis after dental implant restoration

摘要: 目的:探讨牙列缺损患者血清细胞间黏附分子1(intercellular adhesion molecule-1,ICAM-1)、基质金属蛋白酶8(matrix metalloproteinase-8,MMP-8)水平与口腔种植修复效果的关系。方法:选择2021年1月—2023年1月在绵阳市人民医院进行口腔种植修复的80例牙列缺损患者作为试验组,根据其预后情况分为预后良好组(53例)和预后不良组(27例);另选74例在医院体检的健康志愿者为对照组。采用ELISA法检测所有受试者的血清超敏C反应蛋白(high-sensitivity C-reactive protein,hs-CRP)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、ICAM-1、MMP-8水平。采用Pearson相关性分析预后不良与ICAM-1、MMP-8水平的相关性,受试者工作特征(receiver operating characteristic,ROC)曲线分析ICAM-1、MMP-8水平对患者预后不良的诊断价值,通过Z检验比较曲线下面积(area under the curve,AUC)的差异。结果:受试者血清hs-CRP、TNF-α、ICAM-1、MMP-8水平差异表达,试验组患者显著高于对照组,预后不良组显著高于预后良好组(P<0.05);ICAM-1、MMP-8高表达是患者预后不良的独立危险因素(P<0.05);预后不良组血清ICAM-1和MMP-8水平呈正相关(r=0.433,P=0.000)。血清ICAM-1、MMP-8单独诊断患者预后不良的AUC分别为0.776和0.762;两者联合诊断的AUC为0.908,优于各自单独诊断(P<0.05)。结论:牙列缺损患者ICAM-1、MMP-8水平升高,且种植修复预后不良患者水平更高,两者呈正相关,可能协同参与预后不良发生机制。两者联合诊断患者预后不良价值更高,可为临床预后评估提供参考。

Abstract: PURPOSE: To investigate the serum levels of intercellular adhesion molecule-1 (ICAM-1) and matrix metalloproteinase-8 (MMP-8) in patients with tooth loss and their relationship with the effectiveness of oral implant restoration. METHODS: A total of 80 patients with dentition loss who underwent dental implant restoration in Mianyang People's Hospital from January 2021 to January 2023 were recorded as the experimental group, and were divided into good prognosis group (n=53) and poor prognosis group (n=27) according to their prognosis, while 74 healthy volunteers who underwent physical examination during the same period were selected as the control group. The serum levels of high-sensitivity C-reactive protein(hs-CRP), tumor necrosis factor-α (TNF-α), ICAM-1 and MMP-8 were detected by ELISA. Pearson correlation analysis was used to analyze the correlation between ICAM-1 and MMP-8 levels in patients with poor prognosis. Receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of ICAM-1 and MMP-8 levels in patients with poor prognosis, and Z test was used to compare the differences in the area under the curve (AUC). Results: The serum levels of hs-CRP, TNF-α, ICAM-1 and MMP-8 in the experimental group were significantly higher than those in the control group, and the poor prognosis group was significantly higher than that in the good prognosis group (P<0.05). The high expression of ICAM-1 and MMP-8 was an independent risk factor for poor prognosis in patients (P<0.05). The serum levels of ICAM-1 and MMP-8 in the poor prognosis group were positively correlated(r=0.433, P=0.000). The AUC for poor prognosis in patients diagnosed with serum ICAM-1 and MMP-8 alone was 0.776 and 0.762, respectively. The AUC of the combined diagnosis of serum ICAM-1 and MMP-8 was 0.908, which was better than serum ICAM-1 and MMP-8 alone (P<0.05). CONCLUSIONS: The levels of ICAM-1 and MMP-8 are elevated in patients with tooth loss, and the levels are higher in patients with poor prognosis. There is a positive correlation between the two indexes, and they may jointly participate in the mechanism of poor prognosis. The combination of the two indexes has a higher value in the diagnosis of poor prognosis, which can provide a reference for clinical prognosis evaluation.
