玻璃纤维与钴铬合金桩核全瓷冠修复前牙残根残冠效果比较

Comparison of the effect of glass fiber and cobalt alloy post-core all-ceramic crowns on the restoration of anterior teeth

摘要: 目的:比较玻璃纤维与钴铬合金桩核全瓷冠修复前牙残根残冠的效果。方法:回顾2020年1月—2023年12月收治的121例前牙残根残冠患者的资料,分为试验组(61例玻璃纤维桩核全瓷冠)和对照组(60例钴铬合金桩核全瓷冠),记录2组随访12个月的修复效果。比较2组修复效果、美学效果、牙周健康指标、龈沟液炎症因子及并发症情况。采用SPSS 18.0软件包对数据进行统计学分析。结果:试验组修复成功率显著高于对照组(P<0.05)。2组治疗前后的红色美学指数(pink esthetic scores, PES)、白色美学指数(white esthetic scores, WES)相比,差异无统计学意义(P<0.05)。2组治疗后的龈沟出血指数(sulcus bleeding index, SBI)、牙龈指数(gingival index, GI)和菌斑指数(plaque index, PLI)显著低于治疗前(P<0.05),试验组治疗后更低(P<0.05)。2组治疗后的抗肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)和白细胞介素8(IL-8)显著低于治疗前(P<0.05),试验组TNF-α和IL-6治疗后更低(P<0.05)。2组总并发症发生率无显著差异(P>0.05)。结论:与钴铬合金桩核全瓷冠相比,玻璃纤维桩核全瓷冠修复前牙残根残冠可增强修复效果,改善牙周健康和抑制龈沟液炎症反应。

Abstract: PURPOSE: To compare the effect of glass fiber post core porcelain crown and cobalt alloy post core porcelain crown in repairing residual root and crown of the anterior teeth. METHODS: The data of 121 patients with residual root and crown of anterior teeth admitted to the hospital from January 2020 to December 2023 were reviewed, of which 61 patients were treated with glass fiber post-core porcelain crown restoration (experimental group) and 60 patients with cogger alloy post core all porcelain crown repair (control group). The repair outcomes of the two groups were recorded after 12 months of follow-up. The final results, aesthetic effect, periodontal health index, gingival crevicular fluid inflammatory factors and complications were compared between the two groups. SPSS 18.0 software package was used for statistical analysis of the data. RESULTS: The success rate of the experimental group was significantly higher than that of the control group (P<0.05). There was no significant difference in pink esthetic scores (PES) and white aesthetic scores (WES) between the two groups before and after treatment (P<0.05). The sulcus bleeding index (SBI), gingival index (GI) and plaque index (PLI) after treatment were significantly lower in both groups than before treatment (P<0.05), and the experimental group after treatment was more lower(P<0.05). TNF-α, IL-6 and IL-8 levels after treatment were significantly lower in both groups than before treatment (P<0.05), and TNF-α and IL-6 levels in the experimental group after treatment were more lower (P<0.05). There was no significant difference in the total complication rate between the two groups (P>0.05). CONCLUSIONS: Compared to cobalt alloy post-core all-porcelain crown, glass fiber post-core all-porcelain crown can enhance the repair effect, improve periodontal health, inhibit the damage of gingival crevicular fluid inflammation.
