美学区改良盾构术在上颌前牙区单颗牙种植中的应用效果评价

Study on the effects of improved shield technique in the aesthetic zone for single tooth implantation in the maxillary anterior region

摘要: 目的: 探讨美学区改良盾构术在上颌前牙区单颗牙种植中的应用效果。方法: 选择2020年1月—2023年1月上颌前牙区单颗牙种植患者84例(84颗牙),按照随机数字表法分为对照组和试验组,每组42例(42颗牙)。对照组采用传统即刻种植治疗,试验组采用美学区改良盾构术。记录2组患者种植成功率,比较术前和术后6个月患者牙周指标、术后即刻和术后1年红色美学指数(pink esthetic score,PES)、术后1年骨吸收量和植体稳定系数(implant stability quotient,ISQ)。记录不良反应发生率(红肿、出血、疼痛和位点感染)。采用SPSS 26.0软件包对数据进行统计学分析。结果: 2组种植成功率均为100%,试验组术后6个月牙周指标显著低于对照组,术后1年PES、ISQ评分显著高于对照组,骨吸收量显著低于对照组,且不良反应发生率更低,差异有统计学意义(P<0.05)。结论: 美学区改良盾构术可提高患者前牙区美学指数和种植体稳定性,降低骨吸收量。

Abstract: PURPOSE: To explore the effects of improved shield technique in the aesthetic zone for single tooth implantation in the maxillary anterior region. METHODS: Eighty-four patients(84 teeth) who underwent single tooth implantation in the maxillary anterior region from January 2020 to January 2023 were selected,randomly divided into the control group and experimental group using digital table method, each with 42 patients(42 teeth). The success rate of implant in 2 groups was recorded, and periodontal index, pink esthetic score (PES), bone absorption and implant stability quotient (ISQ) preoperatively and 6 months after surgery were compared. The incidence of adverse reactions (redness, bleeding, pain and site infection) was recorded. SPSS 26.0 software package was used for statistical analysis of the data. RESULTS: The success rate of implantation in both groups was 100%. Six postoperative pericrescent indexes of the experimental group were significantly lower than that of the control group, PES and ISQ scores of the experimental group were significantly higher than that of the control group one year after surgery, bone absorption volume was significantly lower than that of the control group and the incidence of adverse reactions was significantly decreased(P<0.05). CONCLUSIONS: The improved shield technique in the aesthetic zone can enhance the esthetic index and implant stability in the anterior region, and reduce bone resorption volume.
