摘要/Abstract

摘要： 目的:评价角度螺丝通道系统基台对上颌前牙种植成功率和美学效果的影响。方法:选择2021年5月—2023年12月收治的102例上颌前牙种植患者,采用随机数字表法分为对照组（n=51,直基台螺丝固位）和试验组（n=51,角度螺丝通道系统基台）,比较两组患者种植体留存率、种植体边缘骨吸收量、美学效果[红色美学评分 (pink esthetic score, PES)和白色美学评分 (white esthetic score, WES)]、牙周指标[牙周探诊深度 (probing depth, PD)、龈沟出血指数 (sulcus bleeding index, SBI)和菌斑指数 (plaque index, PLI)]、生活质量[口腔健康影响程度量表 (oral health impact profile-14, OHIP-14)评分]及并发症发生情况。结果:随访1年,两组种植体存留率均为100%,试验组种植体边缘骨吸收量显著高于对照组（P<0.05）,试验组PES、WES评分显著高于对照组（P<0.05）,试验组OHIP-14评分显著低于对照组（P<0.05）。治疗后3个月,试验组PD、SBI和PLI显著低于对照组（P<0.05）。试验组并发症发生率与对照组相比无显著差异（P>0.05）。结论:角度螺丝通道系统基台可显著提高上颌前牙种植成功率和美学效果,适合临床推广应用。

Abstract: PURPOSE: To evaluate the influence of the abutment of the angle screw channel system on the success rate and aesthetic effect of maxillary anterior implant restorations. METHODS: A total of 102 patients with maxillary anterior implantation who were admitted from May 2021 to December 2023 were selected and divided into the control group (n=51, with straight abutment screw fixation) and the experimental group (n=51, with angle screw channel system abutment) by the random number table method.The implant survival rate, marginal bone loss, aesthetic outcomes [pink esthetic score (PES) and white esthetic score (WES)], periodontal indicators [probing depth (PD), sulcus bleeding index (SBI) and plaque index (PLI)], quality of life [evaluated by the Oral Health Impact Profile-14 (OHIP-14) Scale] and complication rates were compared between the two groups. Results: After 1-year follow-up, the implant retention rates of both groups were 100%,the bone resorption around the implant in the experimental group was significantly higher than that in the control group (P<0.05), PES and WES scores of the experimental group were significantly higher than those of the control group (P <0.05) and the OHIP-14 score of the experimental group was significantly lower than that of the control group (P<0.05). Three months after treatment, the PD, SBI and PLI in the experimental group were significantly lower than those in the control group (P<0.05). There was no significant difference in the incidence of complications between the experimental group and the control group (P>0.05). CONCLUSIONS: The abutment of the angle screw channel system can significantly improve the success rate and aesthetic effect of maxillary anterior implantation and is suitable for clinical promotion and application.
