牙周显微外科在牙周加速成骨正畸中的效果评价

Application of periodontal microsurgery in periodontal accelerated osteogenic orthodontics

摘要: 目的: 探讨牙周显微外科在牙周加速成骨正畸(periodontally accelerated osteogenic orthodontics,PAOO)中的应用价值。方法: 选择100例在正畸治疗前因下颌前牙骨量不足需行PAOO手术的患者,随机分为显微组和对照组,每组各50例。术前利用CBCT测量术区患牙的牙槽骨厚度、高度及牙龈厚度,同时采用牙周探针测量患牙的牙龈退缩高度。术后即刻、3天、7天、14天分别记录患者的疼痛评分,并在术后14天记录创面愈合情况,随访至术后6个月时,再次记录患牙的上述指标。采用GraphPad Prism 5.0软件包对数据进行统计学分析。结果: 显微组在术后不同时间点的疼痛评分显著低于对照组,且显微组患者术后14天时均实现一级愈合。术后6个月时,显微组患者术区牙槽骨厚度增加和牙龈退缩高度改善显著优于对照组,患者满意度更高。结论: 牙周显微外科可辅助提高PAOO的手术效果,具有良好的临床应用价值。

Abstract: PURPOSE: To investigate the application value of periodontal microsurgery in accelerated osteogenic orthodontics (PAOO). METHODS: A total of 100 patients with the need of PAOO surgery before orthodontic treatment due to the lack of alveolar bone in the mandibular anterior area were selected and randomly divided into microsurgery group and control group with 50 cases in each group. Preoperative CBCT was used to measure the thickness and height of the alveolar bone as well as the gingival thickness. The gingival recession height was measured using periodontal probe. The pain level of the patients was evaluated immediately, 3, 7 and 14 days after surgery while the wound healing was recorded 14 days after surgery. The aforementioned indexes were recorded again 6 months after surgery. GraphPad Prism 5.0 software package was used for data analysis. RESULTS: The pain level of the microsurgery group were significantly lower than those of the control group at different time points after surgery. All the patients in the microsurgery group achieved healing by first intention 14 days after surgery. In addition, 6 months after surgery, the increase in alveolar bone thickness and the improvement of gingival recession in the microsurgery group were significantly better than those in the control group, thus achieving higher patient satisfaction. CONCLUSIONS: Periodontal microsurgery can facilitate the improvement of the surgical effect of PAOO, which has a promising value in the clinical application.
