L-PRF联合去蛋白牛骨在侧壁开窗上颌窦底提升植骨术中的成骨效果及分子生物学评价

Osteogenic effects and molecular biological evaluation of L-PRF combined with deproteinized bovine bone mineral in external maxillary sinus lift implantation

摘要: 目的 评估白细胞-富血小板纤维蛋白(leucocyte platelet-rich fibrin,L-PRF)与去蛋白牛骨(deproteinized bovine bone mineral,DBBM)联合应用在侧壁开窗上颌窦底提升植骨术中的成骨效果及分子机制。方法 选取2023年1月—2025年3月在上海市口腔医院行侧壁开窗上颌窦底提升植骨术的40例患者,分为对照组(单纯DBBM,n=20)和试验组(DBBM+自体L-PRF,n=20)。术后3个月及6个月,通过组织学和分子生物学分析评估植骨区域的成骨体积、成骨活性、胶原形成和骨矿化情况,利用RNA-seq与免疫组织化学染色(immunohistochemical staining,IHC)进一步分析相关分子机制。结果 试验组术后3个月及6个月的成骨体积和活性显著高于对照组(P<0.05)。试验组在3个月时胶原形成显著,6个月时骨改建更明显,且形成哈弗管结构(P<0.05)。RNA-seq分析显示,试验组COL10A1显著高于对照组;IHC结果也证实,术后3、6个月时,试验组COL10A1表达均显著高于对照组(P<0.05)。结论 L-PRF联合DBBM在侧壁开窗上颌窦底提升植骨术中能显著提高植骨区域的成骨体积、成骨质量和愈合速度,并通过促进COL10A1表达优化骨修复过程。

Abstract: PURPOSE: To evaluate the clinical osteogenic effect and molecular mechanism of the combined application of leucocyte platelet-rich fibrin(L-PRF) and deproteinized bovine bone mineral(DBBM) in lateral window maxillary sinus floor elevation and bone grafting surgery. METHODS: A total of 40 patients who underwent lateral window maxillary sinus floor elevation and bone grafting at Shanghai Stomatological Hospital from January 2023 to March 2025 were selected and divided into two groups: the control group (treated with DBBM alone, n=20) and the experimental group (treated with DBBM combined with autologous L-PRF, n=20). At 3 and 6 months after surgery, histological and molecular biological analyses were performed to evaluate the osteogenic volume, osteogenic activity, collagen formation, and bone mineralization in the bone graft area. RNA sequencing (RNA-seq) and immunohistochemical staining (IHC) were used to further analyze the relevant molecular mechanisms. RESULTS: The osteogenic volume and activity of the experimental group at 3 and 6 months after surgery were significantly higher than those of the control group (P<0.05). In the experimental group, collagen formation was significant at 3 months, and bone remodeling was more obvious at 6 months with the formation of Haversian canals(P<0.05). RNA-seq analysis showed that the expression level of COL10A1 in the experimental group was significantly higher than that in the control group. IHC results also confirmed that the expression of COL10A1 in the experimental group was significantly higher than that in the control group at 3 and 6 months after surgery(P<0.05). CONCLUSIONS: The combination of L-PRF and DBBM can significantly increase the osteogenic volume, improve the osteogenic quality, and accelerate the healing speed in the bone graft area during lateral window maxillary sinus floor elevation and bone grafting surgery. It optimizes the bone repair process by promoting the expression of COL10A1.
