口腔颌面清创术后软组织感染的相关因素分析

摘要： 目的探讨口腔颌面清创术后软组织感染的相关因素。方法回顾分析我院2013年1月—2016年6月收治的口腔颌面损伤行清创术患者500例,按术后软组织是否感染分为感染组和非感染组,对2组患者性别、年龄、平均手术时间、平均住院时间、抗生素使用时间、简明损伤定级法（abbreviated injury scale, AIS）、是否存在合并伤、颌面部骨折部位、软组织损伤部位、骨折类型和是否有糖尿病史等方面进行比较。采用SPSS19.0软件包对数据进行统计学处理。结果影响口腔颌面清创术后软组织感染的因素为年龄较大、住院时间较长、手术时间较长、抗生素使用时间较长、AIS评分较高、上颌骨受累、唇颏损伤和有糖尿病史。结论影响口腔颌面清创术术后软组织感染的因素较多,其中年龄较大、手术时间较长、AIS评分较高、颌骨受累、唇颏损伤和有糖尿病史可能是影响口腔颌面清创术后软组织感染的独立危险因素。医护人员要根据患者的具体情况给予预防措施,减少感染的发生。

Abstract: To explore the factors affecting soft tissue infection after oral and maxillofacial debridement. METHODS: Fifty hundred patients with debridement were enrolled in this study from January 2013 to June 2016. The patients were divided into 2 groups according to soft tissue infection, 18 cases in infection group and 482 cases in non-infection group. Age, mean time to surgery, average length of stay, duration of antibiotics use, abbreviated injury scale (AIS), combined injuries, maxillofacial fractures, soft tissue injury, type of fracture, and history of diabetes were recorded and analyzed using SPSS 19.0 software package. RESULTS: The factors influencing soft tissue infection after oral and maxillofacial surgery were the aged, longer hospital stay, longer operation time, longer antibiotics use time, higher AIS score, Jaw bone injury and diabetes. CONCLUSIONS: The factors influencing soft tissue infection after oral and maxillofacial debridement are various. The aged, longer operation time, higher AIS score, jaw bone involvement lip and chin injury as well as diabetes might be the independent factors. Health care providers should give preventive measures to reduce the incidence of infection, according to specific factors.
